Bagi individu yang ingin memohon lesen carry & use (c&u) senjata api, perlulah mengemukakan dokumen-dokumen seperti berikut:-

Senarai Semak Permohonan Membeli / Pindahmilik / Memiliki / Membawa dan Mengguna Senjata Api Pistol / RevolverSenarai Semak Permohonan Membeli / Pindahmilik / Memiliki / Membawa dan Mengguna Senjata Api Shotgun / RifleBorang Pol 128Lampiran A kepada Borang Pol 128
Kesemua borang / senarai semak yang telah lengkap hendaklah dikembalikan kepada Ibu Pejabat Polis Daerah tempat di mana pemohon bermastautin.
